The VMD Membership Plan
Not insurance. No claim forms, no waiting periods, and no annual maximum deciding what your teeth are worth.
The VMD Membership Plan is our own plan, run directly between you and this practice. You pay once a year. Your examinations and X-rays are covered for that year, and everything else you have done here is 30% less.
$333 a year — about $28 a month.
What this actually is
Dental insurance was never really insurance. It is a spending cap — one that has barely moved since the 1970s — and it decides in advance how much of your care it is willing to participate in.
This is not that, and it is not a substitute for it.
The VMD Membership Plan is a direct arrangement between you and Vivere Meliora Dentistry. There is no company in the middle, nothing to submit, and nobody to approve it. You join, and from that day your examinations and X-rays here are covered for the year and your treatment here costs 30% less.
That is the whole product. It is meant to be understandable in one reading, because the thing it replaces never was.
What you get
Examinations and X-rays, covered for the year.
Every examination. Every X-ray. Not one, not two — for the year.
30% off your treatment.
Fillings, crowns, cleanings, extractions, night guards — the everyday work of keeping a mouth healthy, at 30% less than our regular fee.
Member rates on Invisalign and cosmetic work.
These carry their own member pricing rather than the 30%. Ask, and we will give you the real number for your case before you commit to anything.
What it costs
$333 for the year. That is about $28 a month, though it is billed once annually rather than monthly.
Additional household members are $150 each, on the same terms. A couple is $483. A family of four is $783.
Join at your first visit and that visit is covered. A new-patient examination here is $199. Join the same day and you do not pay it — members’ examinations and X-rays are covered, and that includes the one you just had. You pay $333 for the year, and nothing for the visit.
Membership renews at $333 a year.
Who this is for
You don’t have dental insurance, and paying full price out of pocket has been the only option.
You have a plan that won’t touch cosmetic work — and cosmetic work is what you actually want done.
You’re self-employed, and buying a dental policy has never made arithmetic sense.
You’ve just retired and come off an employer plan.
If you are already happy with the way your care is paid for, you do not need this. We will tell you that honestly.
How to join
1. Come in. If you are not a patient here yet, start with a visit. You should see the practice before you commit to a year of it.
2. Ask us at the front desk. We will go through exactly what it covers for your mouth specifically, what it does not, and whether it is actually worth it for you.
3. Sign and start. There is a short agreement in plain language, signed on a screen at the desk. Membership begins the day you sign.
Membership is not sold online. We would rather talk it through with you than have you click a button and hope you understood it.

Questions people actually ask
Is this insurance?
No. It is not insurance, not a health benefit plan, and not run by any insurance company. It is an arrangement directly between you and this practice. The full terms are at the bottom of this page.
Can I use it together with my dental insurance?
No. It does not combine with insurance. If you have a plan you are happy with, keep it — this is built for people who don’t.
What is included exactly?
All of your examinations and X-rays for the year, and 30% off treatment here. Invisalign and cosmetic work have their own member rates rather than the 30%.
Does it include cleanings?
Cleanings are 30% off rather than included, and we want to be straight about why. What a cleaning actually involves varies enormously from one mouth to the next. Gum health differs, and some people need periodontal maintenance or treatment rather than a routine cleaning. If we advertised two free cleanings, we would then be in the position of telling some people theirs does not qualify. We would rather discount whatever your mouth actually needs than promise one specific procedure and take it back at the chair.
Does it cover veneers or Invisalign?
Not at 30% — those carry their own member rates. Tell us what you are considering and we will give you the member number for it before anything is scheduled.
When does it start?
The day you sign. There is no waiting period and nothing to activate.
Does it cover my family?
Each additional person in your household is $150 for the year and gets the same thing you do: examinations and X-rays covered, 30% off treatment, and member rates on Invisalign and cosmetic work.
How do I cancel?
You can cancel within 30 days. If you have not had anything done, you get the full fee back. If you have already used the membership for treatment, the refund is reduced by what those discounts were worth. The full policy is in the terms below.
What happens after a year?
It renews at $333. Before each renewal we send you a note showing what the membership actually saved you over the year, so you are deciding with a real number rather than a guess.
Ask us about membership
Membership is not sold online — we would rather talk it through with you. Tell us what you are after and we will call you back, or ring the practice on 904.769.5848 and ask for the membership.
Terms
The VMD Membership Plan is NOT health insurance, a health benefit plan, or a licensed Discount Plan Organization (DPO). It is a direct, in-house discount arrangement provided exclusively to patients of Vivere Meliora Dentistry. Membership covers only preventive care specifically itemized in the plan and provides fixed discounts off the practice’s standard, non-discounted usual and customary fees. This plan cannot be combined with dental insurance, third-party financing promotions, or other discounts. The plan is valid solely at Vivere Meliora Dentistry and pays no benefits or reimbursements to external providers.
Members may cancel their membership within thirty (30) calendar days of enrollment. If no dental services, examinations, cleanings, or discounted treatments have been rendered during those thirty (30) days, the membership fee will be refunded in full. If the Member received dental services, preventative care, or treatment discounts during the initial 30-day period, the refund will be reduced by the full Usual, Customary, and Regular (UCR) fee value of all completed services minus any member amounts paid. If the total standard value of services provided exceeds the annual membership fee paid, no refund will be issued, and the Member agrees that the remaining balance between the promotional member rate and the standard UCR rate becomes immediately due and payable upon cancellation.
THE PATIENT AND ANY OTHER PERSON RESPONSIBLE FOR PAYMENT HAS A RIGHT TO REFUSE TO PAY, CANCEL PAYMENT, OR BE REIMBURSED FOR PAYMENT FOR ANY OTHER SERVICE, EXAMINATION, OR TREATMENT THAT IS PERFORMED AS A RESULT OF AND WITHIN 72 HOURS OF RESPONDING TO THE ADVERTISEMENT FOR THE FREE, DISCOUNTED FEE, OR REDUCED FEE SERVICE, EXAMINATION, OR TREATMENT.
